Post-Surgery Pain Management

Description:

Most surgeons are not applying the latest multi-modal therapy to manage pain postoperatively. This course will instruct general, colorectal, thoracic, and gynecologic surgeons on the use of IV NSAIDS and acetaminophen, gabapentin, and long-acting local anesthetics as adjuncts to narcotic use for managing post-operative pain. A hands-on porcine lab is also provided to gain experience in the administration of long acting local anesthetics using transverses abdominus plane (TAP) blocks (under laparoscopic and ultrasound guidance), and fascial infiltration. These techniques reduce post-surgical pain after thyroid/parathyroid, breast, abdominal, and inguinal surgery.

Course Details & Registration 

 

Description:

The Houston Methodist Cerebrovascular Course is the first Cerebrovascular Course held by the Houston Methodist Neurological Institute. The course is intended to provide training and education to neurosurgical residents, fellows, and physicians in early practice on open surgical and endovascular treatments for cerebrovascular pathology. The course will take place June 12th - 13th, and will consist of lectures (June 12th) and a lab-based portion (June 13th). 

The lecture-based part of the course will consist of a full day of debate-style lectures by some of the leaders in the Cerebrovascular Neurosurgical community on various controversial topics in the field of Cerebrovascular Neurosurgery, in which each faculty member will present their own viewpoint on the same topic.

During the lab-based part of the course, attendees will practice surgical and endovascular techniques in the MITIE Lab under the supervision and instruction of the course faculty. The lab-based portion will consist of two parts:

  • Cadaver head dissection, where attendees will receive training on various open surgical approaches to cerebrovascular pathology and will practice vascular anastomosis on cadaver heads perfused with a life-like, pulsatile blood substitute.  Creation of experimental aneurysms in swine. Attendees will create experimental aneurysms in live pigs by anastomosing a small venous pouch to the common carotid artery under the supervision of course faculty.
  • Endovascular coil embolization of experimental aneurysms in live swine. This portion will take place within the newly-built MITIE angiography suite, allowing attendees to practice these techniques using authentic biplanar fluoroscopy and digital subtraction angiography.

Ultrasound (U/S) guidance for vascular access has been used in clinical practice for more than 30 years. More recently, U/S has been utilized for target vessel visualization during central venous catheter (CVC) access, and multiple studies have demonstrated significantly increased safety, efftiveness, and efficiency using this technique. Despite published guidelines supporting the clinical utility of U/S guided CVC insertion, adoption into clinical practice has been inconsistent with few institutions developing pathways to ensure its use.

A hands-on porcine lab is also provided to gain experience in the administration of long acting local anesthetics using transverses abdominus plane (TAP) blocks (under laparoscopic and ultrasound guidance), and fascial infiltration. These techniques reduce post-surgical pain after thyroid/parathyroid, breast, abdominal, and inguinal surgery.
